Key Responsibilities
- Complete 25+ weekly in-home visits for assessments, diagnosis, medication management, and preventive care.
- Collaborate with healthcare teams, educate patients, and manage chronic conditions.
- Maintain accurate EMR documentation within 48 hours of visits.
- Ensure reliable transportation and participate in after-hours, on-call duties if needed.
- Adhere to healthcare regulations, company policies, and compliance requirements.
Qualifications
Active Nurse Practitioner license with DEA and BLS certification.
Certification as Family or Adult-Gerontology NP.
Minimum 2 years of primary care experience preferred.
Proficiency in EMR systems and strong clinical / communication skills.
Valid driver’s license and car insurance with a clean driving record.
Preferred Experience
Familiarity with HEDIS / STAR measures, home-based visits, and HCC documentation.
