Responsibilities:
- Completes MDS, CAA and care plans within regulated deadlines.
- Coordinates scheduling of RAI process with interdisciplinary team
- Assesses resident through physical assessment, interview, and chart review.
- Discusses the resident’s care needs with caregivers, including physician, nursing, social services, medical, dietary and activity staff.
- Reviews information from hospital, consultation and outside agencies and uses such information in completing assessment and care planning.
- Coordinates, identifies and/or initiates significant changes to the MDS
- Prepares to conduct PPS meetings to maintain MDS assessments according to Medicare schedule and maintains PPS board to monitor Medicare days and RUG utilization in the absence of the Care Management Coordinator
- Remains current with the requirements of the American Association of Nursing Assessment Coordinators (ANAC).
Qualifications:
- Registered Nurse (RN)
- AANC certification a plus. RAC-CT
- Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Perspective Payment Process (PPS) strongly preferred.
- Experience as an MDS Nurse
